In compliance with IRS regulations, form 1095-C is now available in Banner.  For employees that have elected to have printed versions, those are being mailed today.

To opt-in to receive electronic forms:

Login to Banner Employee Self-Service

  1. From the NEOMED website, select Faculty and Staff
  2. Then select Applications Menu
  3. Select Employee Self-Service Dashboard
  4. Click on Taxes
  5. Select Electronic Regulatory Consent
  6. Mark your selections and select Submit
  7. You will receive a confirmation email from Banner that the changes have been made
  8. Note that electronic access is immediate

To access your 1095-C:

From the Employee Self-Service Dashboard

  1. Click on Taxes
  2. Select 1095-C Employer Provided Health Insurance Offer and Coverage Statement
  3. Select 2021 as the tax year
  4. Click Display
  5. Scroll to the bottom of the page and select Printable 1095-C
  6. Enter your password
  7. Print using Ctrl-P
