In compliance with IRS regulations, form 1095-C is now available in Banner. For employees that have elected to have printed versions, those are being mailed today.
To opt-in to receive electronic forms:
Login to Banner Employee Self-Service
- From the NEOMED website, select Faculty and Staff
- Then select Applications Menu
- Select Employee Self-Service Dashboard
- Click on Taxes
- Select Electronic Regulatory Consent
- Mark your selections and select Submit
- You will receive a confirmation email from Banner that the changes have been made
- Note that electronic access is immediate
To access your 1095-C:
From the Employee Self-Service Dashboard
- Click on Taxes
- Select 1095-C Employer Provided Health Insurance Offer and Coverage Statement
- Select 2021 as the tax year
- Click Display
- Scroll to the bottom of the page and select Printable 1095-C
- Enter your password
- Print using Ctrl-P
