AHEAD Podcast: QI Success Story from the Field, Tackling Uncontrolled Type 2 Diabetes, Part 2

In the latest podcast from the Northeast Ohio Quality Improvement (NEO QI) Hub, Scott Yasinow, M.D., and Michael Duffy, Pharm.D., from the University Hospitals Internal Medicine Group in Cleveland, discuss their experiences with the NEO QI Hub's Achieving HEAlth Excellence in Diabetes (AHEAD) Initiative. Moderated by Mimi Singh, M.D., M.S., from Case Western Reserve University (CWRU) and the VA Northeast Ohio Healthcare System, the discussion highlights how closer collaboration between pharmacy and primary care has effectively reduced the percentage of patients with a hemoglobin A1C level above 9%.

The discussion includes key strategies that contributed to their success, such as improved care coordination, enhanced medication management and an efficient referral process. They also address the challenges they encountered and offer practical tips for practices aiming to enhance diabetes outcomes for patients with uncontrolled type 2 diabetes.

This podcast is the second in a series showcasing the successes of participating practices in the NEO QI Hub’s AHEAD Initiative.
