To ensure the safety and security of the University community while minimizing adverse effects on individuals traveling abroad (outside of the United States), the following form can be used to notify the NEOMED IT department of forthcoming travel where access to a NEOMED account is necessary.
Upon the completion of this form, your information will be reviewed, and steps will be taken to ensure your account is enabled during the period of travel. You may be contacted with additional questions if clarification is needed.
