Cityblock Health is a novel community-based care delivery company in New York City, NY, designed to serve populations with complex needs. It partners with community-based organizations, health plans and clinicians to tackle challenges of urban health. Cityblock focuses on providing access to integrated primary care, behavioral health and social services for Medicaid and low-income Medicare beneficiaries with the aim of building a scalable solution that addresses the root causes of poor health in underserved populations. In addition to care delivery services, Cityblock leverages the power of technology to codify workflow and enhance communication between care teams, members and the communities in which they reside to achieve shared goaols and drive action and accountability at scale. Lastly, Cityblock's Neighborhood Hubs are living, breathing spaces for caregivers, members, and local organizations to meet, engage, and execute on the many factors that affect health at the local level. Cityblock customizes its Hubs, community by community, to meet their needs and support their vision.
