A reminder that we will hold a flu shot make-up clinic. This will be the last clinic held at NEOMED. You will still be able to visit the pharmacy and request a flu shot from one of the on-site pharmacists. Other options are still to visit your PCP or a local pharmacy in your area.
WHEN: Tuesday, Oct. 13 from 1-4 p.m.
WHERE: Old Pharmacy location in the NEW Center
As a reminder:
- Face covering and social distancing should be practiced at all times.
- Review the Vaccine Information Sheet (Flu VIS.pdf) and Fill out the attached Vaccine Administration Form (Influenza Administration Form 2020.pdf) in its entirety no sooner than 24 hours before your scheduled appointment.
- You will be asked to wait outside on marked lines and comply with proper safety practices if at any time the clinic room is at its max capacity .
- Follow one-way traffic pattern to enter and exit the flu clinic as marked.
You will be required to provide a copy of your completed immunization record to Human Resources for tracking purposes. Note that obtaining the influenza vaccine is a term and condition of employment so failure to comply with the mandatory vaccination requirement will result in disciplinary measures detailed in the employee Social contract agreed to upon your return to campus (link below for reference).
