As a part of the Wellness at NEOMED Faculty and Staff program, those enrolled in NEOMED’s Health Plan, both employees and spouses, are able to earn a discount to the employee contribution costs effective January 1, 2019.
Discount Qualifications
To receive the Wellness Discount participants must accumulate 400 Wellness Points before September 30, 2018 in the following categories:
Qualification 1- Physician form (50 Points)
Appointment by September 15, 2018
Form due September 30, 2018
Do not wait to schedule!
Qualification 2- HRA (50 points)
Will open to all employees Sept. 1-30, 2018
Qualification 3- Participation (minimum of 300 points)
Completed through portal or participation in any Wellness at NEOMED Faculty and Staff program
