AxessPointe participation in the Ohio Medication Therapy Management (MTM) Consortium, sponsored by the Ohio Department of Health, was published in the Journal of Primary Care & Community Health. Improving Chronic Disease Outcomes Through Medication Therapy Management in Federally Qualified Health Centers features the interdisciplinary practice model at AxessPointe along with two other Federally Qualified Health Centers in Ohio and its impact on improving diabetes and hypertension management.
